How to Make Your Food Log: (Table 1.0)
Download the food log template. Record everything you eat in 2 24-hour periods, carefully noting amounts. Remember to keep track of beverages and condiments, too. The more accurate you are in recording amounts, the more accurate your analysis will be. For help in judging portion sizes, the Mayo Clinic has a nice slide show (Links to an external site.) to help.
How to Make Your Physical Activity Log: (Table 2.0)
You should complete your physical activity logs for the same days you record your food logs. On the activity log found in Table 2.0 in the attached template, write down every activity you do from the time you wake up to the time you go to bed on one day. Include work and recreational activities.

Identify Key Food Finding:
Use one of the websites listed below to analyze your food and physical activity, and choose 1 key finding from your diet analysis. This key finding should show either too much or too little of a macronutrient (carbohydrates, proteins, or lipids). If you did not get too much/too little of a macronutrient, choose the macronutrient that was closest to the minimum or maximum threshold. Your finding may be a graph, chart, or table that shows that you got too little or too much of the macronutrient. In the body of your submission, explain what you over/under did it on and what food possibly led to this finding.
